I had this issue and had tried everything I could find online! For me, the real issue was one of my external monitors. If you have a USB based external monitor/video card try uninstalling the driver. I tried turning off the monitor and also deactivating it but those did not help. I have to keep the actual driver uninstalled to allow FO3 to run.

PS: This is on a HP HDX 18t laptop using the nVidia GT 130m mobile graphics with Vista 64 and now Windows 7 64.
