It was annoying to get the dead ghouls, followers and hubbies endings each time I finished the game.
 
Followers will die all the time. Originally there was a mission so that they won't die, but it was cut.

Dead ghouls can be prevented by fixing their water thingy.

I recommend you don't visit The Hub after exploring around the wasteland for 200 days. Same goes for Shady Sands.

Even if the vats are destroyed within the initial 150 time limit?

I'm not sure about that, but I'm guessing yes.
 
Thank you, I just prove it, I think they die instantly after destroying the vats, I get the good ending as long as I don't re enter the map.
 
My guide has the exact conditions for each ending, so if it doesn't say anything about destroying the Vats or killing the Blablaspoiler, then those don't factor in.
