Yes, I know this has been done to death; but it's my turn now. Prepare for some stupid ideas.
1. Aliens - Ever wonder where the wannamigos come from? Are they truly aliens, or just another warped FEV creature menace. Here's one theory.
The truth in this version of FO3 is that they are an alien version of pets. They were sent down to Earth to scout the planet to see if it was worth invading. Their masters using their advanced alien technology learned of the nuclear disaster that befell Earth, and they plan on finishing the human race off once, and for all.
At the conclusion of the game, you confront the alien predators, and proceed to dela with them. Otions include blasting them out of the sky, convincing them the human race isn't worth the effort it would require to squash them, or negotiate a truce.
The main reason for such a story is to explain the existence of the wanamigos, the alien blaster, and the alien ship that seems to be recurrent in the FO universe.
2. Eurpoean Evasion - While the first two games were sent in the Southern States; FO3 could be set globally which would mean a much longer gameplay.
In this version, 3 large factions (2 human & 1 mutant) are vying for control of all Europe, and subsequently the entire world. They all have dreams of conquest, and versions of what they think is "right". You can choose to side with one of the parties, none of them and creating your own faction to combat them, or negotiating a peace treaty. Once again, this gives an oppurtunity for multiple cchoices, and endings.
3. Return of the Enclave - Though the PC successfully dealt with the Enclave in FO2, there was a secret base that was never discovered. This base has since grown, and the new leaders are now seeking revenge on FO2's heir and countries. Unlike the previous incarnation of the Enclave their goal is not to bring unity to all "humanity"; but unpure revenge. The PC has to look for help before the Enclave annexes too many territories which brings into play the PCs negotiating skills, or they can be dealt with commando style.
Hmmm... These are ideas are just bare to the bone; but I can elaborate on them if people are interested in more detailed information. I don't know about the alien one as it is out there; but it's most useful explaining all the alien innuendos in the previous games.
