A friend of mine had a similar problem years ago and it turned out he had a pirated copy of Fallout,,,
Really, it sounds that you have a copy, in which all the "extra" (cut-scenes and death animations) have been removed to make the game's file size smaller.

Anyhow, not a pirated copy.
It is, however, an install from an iso I made, so that might be the issue?
(I make iso's of all my game discs, then store the actual discs and run the games from iso's)

Try installing from the CD, most versions of Fallout run well w/o the CD in the drive once you use the Humongous install option (which i assume you've not used, since it would be a profound waste of space with an ISO image)

I really can't see how installing from an iso would remove blood and cutscenes and leave everything else intact.
