A question about the past

I realize that the Holy Hand Grenade is impossible to get without an editor or doing that Shift+F5 thing. I came to find this board a little while after all that stuff blew over. That is why I would like to ask how people thought you would get the Holy Hand Grenade. Me and my friends think that it is interesting how almost every game has at least one rumor, and multiple ways to get that rumor. I don't mean this message to stirr up anything. I just would like to know. Thank you.

You were supposed to be able to get it in a random encounter, but it was removed. But the memories never will be...

Let's see, there was the Elvis atomic bomb BFG thing a while ago, then there was the infamous Camikaze (seriously fucked up guy), but BOS Steel Knight is the one we all remember...
