Hmm, as i recall, you need 75% doctor skill (if you play on normal/hard - you can get +10/+20 by setting the difficulty to easy temporarily) and just log into the vault city vault medical computer (no need to use any skill on that) and search the database "for anything interesting" . If you don't find anything, you need more Dr. skill. After that the doctor in the courtyard can perform the operation (fix the auto-doc first or you'll die) provided that you have a shitload of cash & combat armor. The San Francisco Dr. performs these operations also. You can have 4 operations in total, the first 2 are harmless, but the second 2 drop your charisma by 1 each. you get +5 to dmg resist or some fire-explosive resist on each operation, so think carefully if the charisma loss is worth the gain on the second operations.
