how do you pull a tube out of an ass?

You know what I'm talking about. I cant figure out what I'm supposed to do to save this guy. My doc has high skill, but fairly low perception. I've tried pumping him up with mentats, but I still cant figure out what to click to get him to take the damned tube out. If you can help me, I'll give you a cookie.

He can't be saved - kill him by pumping him full of poison. But there is a trader in a special encounter that has a pair of brown forceps, I guess you could try targeting the forceps on the groin. (j.k.)
