karma reset consequences ?

First time out of the vault
I`m trying to find a way to level up past lvl 20, so it is possible i know with commands or mods, you can even get skill points past 100. but my concern is that even if i have still very good karma level the title is back to vault dweller. So please explain me what are consequences for me if i want to play further in the game(main and minor quests) with this status ? I know this title has some connection with what this guy at GNR is talking about me, so he stops talking about my good deeds ? And what about the game ending is it connected to karma title ? pls explain it to me if i am missing something...

Note : i dont think set level gives you the chance to distribute skill points and such. I used the set level command after console command all my skills to max. Set my level to like 50 so id never get enough exp to level again and get stuck at the point distibution screen.
 
Yeah i know there is no titles after lvl 20 but after lvl 20 i should have the highest karma title not reseted to the karma title from 1 level ? I know this is some problem to the modders that it reset to 1 level title but my question wasn`t about this but about consequences of continuing to do quests and finishing the game with this title
 
come on guys :) someone has to now this :) i`m near lvl 20 and i want to continue leveling past it but dont want to ruin my ending or something ?
