So I got the elevator working and went down to the level that Barnaky is on. He has been turned into a machine. As soon as I leave the elevator and go into the room he says his speech and sends all his biches to come kill me.

I heard about multiple endings and stuff. I have the letters from him to his wife with but but how do I use it?

Walk up to him or let him come to you. Shoot him a couple of times and he will come for you. Then he sees it. You do not have to have it in an active item slot. And he leaves the battle. You still have to contend with the other bots though.
