Mission fails??

First time out of the vault
Seems everytime I try to finish a mission it will say complete, the objective window. After about 10 seconds another objective window will pop up saying i'm dead.

What's the deal?

Ive tried a few patches and I also wanted to use a no cd patch since I like to keep my fallout 3 disc in the system...

Anyone ever experience problems like this before?

PS.

I did manage to get past the first mission but after I killed the first enemy in the freeport mission I got the "you died" pop up again.

Clearly I did not die!
 
No are you using drugs, because while clearly you didn't die you've offered no proof that your main character didn't.

Supply some more details, screenshots etc on your character, squad make up and the exact circumstances when the objective complete and failure messages pop up.
