First time out of the vault
I've seen a few usernames here and at the Beth FO3 boards that had "None" in them. I can only assume that this is a reference to the default name in Fallout. I can't tell you how many times I've started a game of Fallout, only to learn an hour in that I spent about an hour working with SPECIAL while completely forgetting to enter a damn name.

Is this a common occurance?
 
Cut it out. Yes, this is a common occurrence. So much so that it was iconic in the early days of the Fallout community.
 
It never happened to me. Maybe it's because I usually start the character creation by giving my character a name :) .
 
That's never happened to me, though I'm quite scatterbrained most of the time.
For instance, I'll always input my name, mess around with the perfect SPECIAL setup for this playthrough.. and then forget to change genders when wanting to play a female character.
So up comes this musclebound man with a feminine name.
 
Yeah that's happened. Not for a while tho. I've learned not to do that. But I used to do it all the time back when I started.

I think about my first 5 playthroughs of Fallout 1 and 2 were under the "None" alias. Interestingly enough, my first character had intelligence of 2... so the name "None" and the limited dialogue mixed together with sometimes hilarious results.
