Unexplained deaths of team members

This has occured a few times now - once I complete a mission or a task from a mission, one of my team just up and dies for no reason. Is there a fix for this? This usually happens right after a message screen.
 
Yes - FOT and latest patch is installed. Does patch 1.27 have all the fixes version 1.25 had? The character is wearing power armor and was using the bullhorn trick which makes them have a buffout addiction. The character (Brian) dies off when he levels up (At Newton when the EMP gun is retrieved from the safe). His HP are at 247 right now. Is it possible to hae too many HP when you level up which would make the character die?
