Probably a sawed off shotgun. You know, mirroring this Mad Max picture:

I don't think the 14mm pistol has that long of a barrel.

The first time I had seen it, I thought it was a pistol of some sort, probably 10mm, but it is too big, so I was wrong.
It's a sawed-off shotgun.

There are some other shots of the Vault Dweller in the manual.
In one of them, the weapon can be seen from a different angle.
I'm pretty sure it's a shotgun:

He has a combat knife, right?
Also he says he had a "really big gun" when talking about his journey to crush the Mutant Unity. Shotgun doesn't look that big :/
Even though he has it on almost every picture.
