There's not too many diseases that could make it in to a vault that would spread fast enough to where the population would be stricken before it was detected. Remember, vaults have their medical bays at the very front of the facility so that when you enter them, you can be examined for diseases or radiation poisoning. That's the whole point of keeping that thing by the door.
