It Wandered In From the Wastes
The only real bug I've encountered so far (PC version) was shortly after entering Springvale. I was wandering around looting houses, and noticed a woman in leather armor and armed with an Assault Rifle hoofing it through town at a rapid pace.
I saved and spent several reloads trying to attack her for the stuff she was carrying, and one of those attempts involved a baseball bat.
I connected with the bat in V.A.T.S....and my character ended up twenty feet in the air, tangled in the power lines. He just kinda floated up there in between swings.
