I would assume that is his birthday cake cutting wheel, as its a bit big for surgical work on anything smaller than an elephant.

Or brain surgery! A big sawblade with lots of small teeth is pretty good for cutting a head open. The small teeth provide a nice clean cut. Then you can access the brain and do... whatever you want! Muahahaa!
