It keeps about 70% of the original story with the other 30% being either new scenes or seemingly pointless changes.

For example, A trip to the Country has Salvatore speak exclusively Italian in it which I find to be a good change yet they also turn it into a shooting gallery at the end after you steal the documents. Also, Sam is the one who suggests you spare the hooker in the hotel instead of Tommy deciding to do it of his own volition but Sam is still like "you spared her bro, gotta put you down" at the end of the game.

Yes, the whole thing being that Sam is completely irredeemable. Given he's based on Robert Deniro's character from Goodfellas, that should be understandable. The games never wanted to glorify the mafia and making it so Sam was never actually your friend isn't a bad decision in my mind.

It's way less dramatic too. They don't even get winged by the train, their tire erupts and they crash into a tree.

Honestly, I hate how much they butchered You Lucky Bastard. The whole game has some differences to it's missions, some I agree with, others not. But they completely ruined that mission by cutting out about 60% of the content. I guess modern gamers are too hyper to sit and wait for a minute or two within cutscenes...

I admit that was my least favorite mission by far. The new version is just wave after wave of enemy in a game normally lacking that.
 
Been playing the game a bit since I bought it recently. Still rather skeptical of many of the changes to the plot but I guess I've warmed up to the whole thing. Still rather unhappy about the changes to You Lucky Bastard but the rest of the game is still pretty good.

I feel like the vehicle list is a tad smaller than the original though, unsure if they were just heavily redesigned or cut entirely.
