Alright, to actually answer the whole point of this thread, I have encountered this "bug" before. Luckily, I saved right outside the entrance of the Military Base and did some experimenting. I may be wrong, but I believe your character HAS to be the one that kills the door guard (the black-clothed mutant in front of the base's door). I killed him and the holotape was on the body. You stated that you had the Crack Assault squad with you, as did I. I think that if one of the paladins KILLS the door guard, then the holotape doesn't drop on the body. I'm not sure if this is legitimate but that is what happened with me. Hope this helped, I'm only a newbie so don't take my word for it, haha!
