Really need someones help

First time out of the vault
So I have been playing fall out 1 and 2 recently for the first time, I was experiencing lagy mouse and jittery scrolling of the map (when you use either the arrows or put your mouse on the map edges) but then I watched a you tube video of someone else's gameplay and it is extremely smooth. Not only that but my conversations/barter is so slow at times it makes the game take much longer then it should. I'm using killaps unofficial patch and high res so after some research I deleted ddraw.dll and that fixed the mouse problem but the rest still are there. I have a great pc a i7 16gig ram and a gtx 1060. I cant seem to solve this and now that I know what my gameplay could be like its ruining the experience for me :( sorry for the wall of txt I just realy need help Sincerely WiseGuy
 
Another very odd update to this, so I noticed that when I put my mouse cursor on the bottom of the screen near the options menus and the gun selections and what not and I scroll the map from left to right I get no lag no jittering and no screen tearing. This is completely blowing my mind and I really want to enjoy this game it seems amazing and its my first time ever playing them. I'm really hoping I can somehow find a fix. I'm new to forums as well but I heard nma is very helpful when it comes to this I'm hoping to bump into my knight in shining armor haha. Thanks again in advance
 
Thanks for the assistance,I tryed all 3 of those things and all of the options involved, it’s so strange and if found a few other posts that seem to have the exact issue I have, for example if I’m in the select load game and my mouse cursor isn’t on the actually box my mouse experiences no lag similar to the main menu but once I got to select the save it gets jittery. Iv tryed a fresh install before patching and what not and that makes it jittery all the time so the patch is definitely helping just got this last hurdle to pass. It’s a shame I wish this tearing and slow down didn’t bother me so much because this game is amazing :(
 
Also is windowed mode suppose to put it in a actual window cuz it basically just stays full screen no matter what
 
There can be fullscreen windows; it's different from exclusive full screen.

Are you running any kind of capture software? (or some driver feature that keeps the last 30 seconds of gameplay video?)

Fallout will run on any PC from the last 25 years... Do you have, or know anyone who has an old (junker) PC?
If not, thrift shops sometimes have them for a few dollars. You can use it to play Fallout 1, and 2—and FO:Tactics.

Alternatively:
If selecting all of the Windows compatibility settings does not alleviate it, Fallout does not actually need or use a 3D accelerator... so it can fully run under limited emulation. You can download Virtualbox, install it, and install a free version of Linux into Virtualbox (and install Wine to run the game), or use an older widows CD/DVD if you have one. That will get you a virtual junker PC for free.

Thanks for the response I’m going to try the emulation route now, I don’t have any type of software running in the background and the game itself runs okay besides when I leave default setting(no high res patch) or when my mouse cursor is on say the load menu it was start to lag/hutch like I can’t make a smooth circle but above the actuall menu where he black area is it runs fine lol. Then the scrolling is slow left to right however if I leave the cursor on the bottom of the screen where the menus and hand select is it can scroll fine. When I try to use compatibility mode it pretty much always fails to launch, I do have software where I can record if anyone’s interested to see it, other posts explain it even better then I but there is never any answer on a fix. Thanks agian though I’m considering reformatting because I want to play it perfectly lol

Edit also fall out tactics seems to run fine
 
Update: So I got the idea to try the game in safe mode and sure enough it works fine, so I'm thinking some type of driver issue.
