IF you can kill in one shot, when no-one else is around you can get away with it. For example, using gore, with slayer and silent death i could kill super mutants who had 88ish Hp in one attack and then run up behind another and do the same. Also using the pancor shotgun with fletchette rounds i could kill muties quick without being detected!
Therefore, i think you could stay sneaked if no-one else SEEs you do it and they die without knowing you did it.
 
Well the game boots you out of sneak mode if you fire a weapon, even if you are the only one on a map.Of course if there's no one in range to detect you you can start sneaking again right away. But if there is someone in range you can start sneaking again quicker if you've only fired a single shot rather than a burst.
