When does the bonus mission take place?

Does it happen after a certain mission? Do you need to have a certain amount of exp. or rank? Can you go back and play it, or when its gone its gone?
 
i got it after finishing quincy, so it's gonna be 5th mission in the game, i don't think that u need any special exp or rank to get it.
i'm stuck on the damn thing cuz of the invisible brief bug...
i can't see objectives
there is nothing special about it

first time I got it I installed it later in the game, I got it immediately after I changed bunkers. 2nd time I got it was at the same time as Voodoo did, right after quincy, same time as mordin. I tried doing mordin first and when I whent back to the bunker it told me to move to the new bunker, but I had to do springfield first.
