I often see people with coloured in radiation symbols under their profile pic, how do you get that?

Your custom user title replaced the trefoils. Sadly, you can't have both.

I'm guessing you just have to go and keep getting rad signs and eventually the more you get, the more radiation symbols you get over time.

No, it's based on post count, and as Hass said, if you have set a custom user title, it shows that instead.

Just remember that once they go up, just as real radiation, it will start to go down again. But you will get different messages.
