A: Sometimes they don't, sometimes they do. When they don't, they come after your guild. So, if you come "home" one day and everyone's dead (that actually happened to one of the play testers), it's not a glitch; it's something you did.
So, if something happens in the game world, it is a direct consequence of the player's actions?
If that sounds too fancy, here is a simpler explanation. Some events will require your active participation and will not start until you sign up (a typical quest). Some events will take place with or without you, triggered by different things (time, you leaving a town, betraying a faction, etc). Without your involvement such quests will be solved automatically and a pre-defined outcome will be selected. With your involvement, it's completely up to you how you handle them. Something like that.
