I just realised how unoriginal Bethesda is...

I don't see how having more than one vault dweller character is a problem. There's more than one good way to do it. It's not as though the Vault Dweller's backstory was so interesting or unique. It was basically just a blank slate, which is from a design standpoint was kind of necessary for it to be an rpg. Which it should be, but isn't always. Hence some 'original' pc backstories being in shittier titles, both with the phrase 'Brotherhood of Steel' in them. Different vaults as we've seen vary greatly. At their heart they are just bunkers. A basic type of building. Culturally, ideologically, societally, two vaults may have no more in common than they do with other factions on the surface. The bungling comes in via other means, like recycling every old ideas they could find for Fo3, and just overall not really being an rpg like Fo4. That they both have vault dweller pcs isn't really the issue.

Ideally what I'd like to see going forward is actually the same thing over and over again. You play as your ancestors, maybe multiple generations, making key decisions at different periods before, during, and/or after armageddon--or for yourself if you choose to become a ghoul. Basically just a series of scenes that could convey a lot in a short time span, including the dramatic changes to the world. While determining where you start, your background, starting gear, that kind of thing. Whether you're a vault dweller, townie, scaver, raider, nomad, etc or some faction specific to the game it'd be your choice.

My issue is that Bethesda never changes, just like war.

Its ALWAYS "you iz a prisoner" for Elder Scrolls, and now its "You iz a vault dweller" for Fallout...

At least 2 and NV gave you different starts!

Fallout 4 uses a Pre-War character, which is different, but honestly what does it add to the game? You could easily take away the whole Pre-War element and you could still have the story left intact bar a few minor tweaks.

Its just lazy, who the hell has a franchise where every game starts the same? "You iz in vault, you leave vault"?
