They do, and they don't. They're inconsistent. Sometimes stealth skills will net you 0 Infamy, and other times you'll go from Neutral to Villified after 2 separate kill shots. I've encountered all across the board responses to Stealth Kills, but I find that, since I lean so heavily towards sneaky sniping, that I still get away with it more often than I don't, so I keep Stealth Killing.

Lots of the Legion Assassins sent after me coincidentally spawned while I was crossing the El Dorado Dry Lake, and they couldn't seem to walk through it due to the ants, so I just kept myself surrounded by Neutral-Hostile critters and they could never seem to close the distance between us. Another time they ended up spawning near the Grub 'n Gulp and killed Fitz (who took a rather proactive stance on "defending himself") so I reloaded and hunted them down before they could attack him. I sniped the leader before he reached me, then the rest of them just stood around, dumbfounded and confused as to what to do next, so I picked them off one by one and then had their belongings shipped to my suite for future processing... =)

The same thing happened to me and I did the same basic thing. I sniped them with a AT rife and explosive rounds and then it was all over I give Fitz a legionaries' bush gun in case it happened again.
 
You have to stealth kill them with a silenced weapon. Then, it's not supposed to affect your rep. With certain individuals, even this does not work. In Cottonwood Cove, when I stealth killed Cursor Lucullus (the ferryman), I was instantly "Villified". I reloaded my save and tried different silenced weapons and range. That didn't work either. Then I brought a companion along and punched Lucullus in the face. My companion killed him and my rep stayed the same.

Back on topic, on my last playthrough I've was having the assassins just start shooting from a distance. They didn't run up and announce themselves like on previous playthroughs. Very annoying.
