Samurai blades/scottish claymore.. If anywhere in the game maybe some shelled out museum. Though not a weapon you'd find laying about the wastes in wide use -- pretty impractical -- Think of that scene in raiders of the lost ark when that guy was twirling around his falchion and indy just clocked him with the revolver.
Guns: a grenade launcher would be awesome! maybe a paintgun or something as a joke,does 1-2 damage,blinds if shot in the eye.
that's a little overpowered for fallout's world and not very practical for an RPG.

You can already add your scope to the hunting rifle. If you mean an item system like Arcanum's with a facet for gun smithing yea that would be interesting.

Who's manufacturing it? it wouldn't be anything to advanced probably some sort of patched together leather armor or very crudely welded metal armor. If you're thinking combat armor or PA you can forget about it. There's no one that made it before the war, and there sure as hell isn't going to be someone making it afterwards.
Drugs:
Cigarettes would be sweet, raises your agility, poisons you after you smoke so many.
How would cigarettes raise your agility? I'm baffled by the logic on this. Clogging up your lungs == increased aerobic activity? You're silly. The only conceivable thing I could imagine for cigarettes would be +1 chr.
A wearable stimpack of some kind, costs you a couple of action points, pumps you full of endorphines and all that good stuff, think Predator.

Poison, in liquid form, have a bottle of the stuff, go slipping it into people's drinks, maybe even coat your weapons with it.
coat your weapons yea, great so when you try to handle them you poison yourself LOL!!!! That's a bit fantasy-esque.. poison tipped arrowheads and shit like that. Kind of silly for fallout. There's already poison's in FO2. Remember the mutant serum? Or the poison you use on westin? I remember poisoning lynette too.
Environment :
More interactive environment, if a guy is standing beside a gas can, you can shoot the gas can so it blows up.
Sounds good, though what the hell is a can of gas doing in fallout? did you miss that whole intro movie bit? A can of gas is WAY too valuable to leave laying around for some shit head to shoot.
If there is a vent say, you can climb up it and spy on people,shoot them.
You've been playing too much splinter cell.
Crouching while shooting, crouching lowers your action points but gives a bonus to your armor class.
Stances have already been suggested and were implemented in FOT. I'm sure whoever/whenever FO3 is created will have this in.
Aiming, I think this can be expanded, punch a guy right in the jaw, aim a gun at someones kneecaps,shoot an ear off,lol.

NPC's:
I think F02 got it right when they gave NPC's alot more personality, it would be nice to know what happens to them at the end of the game. Like the blurb about Myron from FO2.
I always...ALWAYS...kill myron.
I think being able to totally customize an NPC is a bad idea, but maybe having unique racial traits or being able to assigning them unique skills.
I don't like this idea, an NPC should exist independent of the PC. Someone who sees you on your quest thinks it noble and decides to join you.
Super mutants having a strength of 11 or something, robots have greater damage resistance, ghouls light up at night =)
You already had a super mutant in FO2. Same as the robot though I'd change this over for the engineering/science character making those skills more useful. Ghouls...to radiate you. already done in FOT and they're not terribly useful allies. Slow moving, neg reactions from pretty much everyone else.
For skills, say :

This is silly, this is like the dermal impact plates and shit from Fo2. I don't like this bionics crap they're adding. it seems superfluous. Like sci fi stuff without cause for being in the universe.

Though the standard deathclaw limb is more than capable of dismembering most anything,occassionly thought it isnt enough, replacing a talon with our steel ginsung one ensures a clean cut everytime.Heck it will even cut through a steel pipe without dulling, guaranteed.
Friendly deathclaws are all dead. The Enclave toasted them. Forget about talking deathclaws. They're out to kill you.
Character:
This refers back to the npc section a bit.Race would probably be out of the question for the main character, human is the standard,still I wouldnt mind playing as a mega-fist wielding super mutant though =)
customization of character / randomizing start ((back story)) would be interesting to do in the fallout environment but it sounds more like you take every game you play and turn it into quake. I wonder if you've ever played through fallout and REALLY explored the non violent routes.
I'd for the more geeky RPG approach, being able to choose build type, ie height weight,face etc, maybe race, though it probably isnt relevant.
This could be interesting if they made it relevant.
More equipment slots, say a belt, to holster your mauser 9mm, it'll cost you less to draw than going to your normal equipment.
What? Fallout 2 wasn't monty haul enough for you already. What the hell are you going to do with more inventory room.
Nothing like customized boots or anything, just useful stuff, shades maybe ( shoot 'em up in style =)

Story/towns/years:
It definitely has to have a wicked ending boss,I like what other people were saying about the Master coming back (thought how he survived a nuclear blast is besides me) I'd like to see Harold talk to him again, see if he remembers or something...

Also the lieutenant of the army base from F01, he didnt really die (Tony Jay does the voice btw) he is free from the master's control but still wants to lead mutantkind to "greatness", maybe he's hideously scarred or something and pissed,hehe.

I definitely think fallout is getting too civilized, too many cities,advanced technology etc. it would be interesting to visit some of the old locations too.
Wow, this is probably the best bit of post. I wish you'd keep this line in mind next time you begin your post. Keep this in mind and read the beginning of your post -- grenade launchers, bionics, etc. Silly no? I'm not trying to flame but its funny that you should "get" fallout after suggesting all that non fallout stuff.

hmm, no. It would have had to have been a mutiny. I can't see anyone reasonable storming the BOS base. 1) there isn't the tech outside of the BOS base 2) there isn't the military tactics/discipline of the bos outside. How about places like NCR destroyed. Broken apart by its overpopulation in a resource scarce environment. -- how exactly DID they feed all those people?
The story should take place after F02 (but a prequel appeals to me too).The great, great ancestor, or maybe a complete stranger, something different here would be good.
I have no opinion on this, both a sequel and prequel have their pros and cons. Depends how its done though it should definitely stay in the vault lineage.
Special encounters:
They probably wont be done the same way, but I've always wanted to see one of Inspector Gadget, it was a great cartoon before the movie bombed the concept all to hell.

you find a man disguised hydrant or something that talks to you and give you an assingment, hands you a piece of paper, you can read it later, or crumple it up and throw it away and it blows uo,lol.
