First time out of the vault
I haven't made a suit, but from what I've gathered it would be pretty simple.
There are two versions to work from. There's Vault 13:
And Vault 101:
Despite my purist instincts I'd have to say that Vault 101 is aesthetically a better looking choice for a costume. There isn't much of a difference, besides the fact that 13 is tighter, has rubber boots, and less detail than 101. The tightness and boots, however, would be unflattering on most. So I'd go with 101.

To make it a bit more like the Vault jumpsuit, I'd alter the collar by shaping it around my neck and sewing interfacing on the inside so it would stay stiff. I would also take out the buttons and replace it with a zipper, as it seems there's a zipper down the front of the jumpsuit.
At the end of the sleeves are leather cuffs. It doesn't seem to be necessary but recreating it would be fairly easy. You'd take vinyl or faux leather (or real leather if you wanted to be fancy) just cut it to size and sew it on the cuffs.
Then I'd sew yellow strips of fabric or ribbon down the sides of the zipper and around the collar. Ribbon seems to be a better choice, because if fabric isn't properly sewn on it frays easily.
On the collar is the number of the vault. For that I would suggest fabric pen, since it's so tiny. Maybe make a stencil first.
For the accessories, I'd first buy a utility belt. Although the belt in the picture doesn't appear to be a utility belt, using such a belt for the costume would be wise because it can hold stimpaks, buffout, and the like. It also looks pretty snazzy. For the boots, I'd say boots that look like this:
Such boots can probably be found easily online or in a consignment shop.
So that's what I'd do for a basic Vault costume. You know, after typing all this I might actually want to start on one. Don't have the cash to buy all the supplies, though.
