And doesn't have quests, towns or any other human beings unless you have the millionaire mod
 
I don't see a major problem with randomly generated encounter areas... really, the only difference between that and the original Fallouts would be that there would actually be some variety to the encounters rather than having them play out on the same four or five endlessly circulated maps.

If you made each "key area" a respectable size and gave them large outskirts areas that were pre-desisnged, and then added in a map screen for overworld travel with randomly generated encounter maps, you could have the best of both worlds-- a meticulously planned and plotted game with plenty of interesting storyline quests and detailed exploration and a total "pre-determined" map area equal in size to New Vegas or even F3, as well as a game with realistic overland distances, plenty of faffing about and killing random life forms (if you're into that) and a total gameworld size equal to that of the original games. You could even bring back the luck-based (or perhaps now, luck-, survival-, and/or perception-based) special random encounters from F1 and 2.
 
Well if you think about it. It's not very far from Vegas but it is kinda isolated. It was more of a transfer station. It didn't hold many people simply because any one there probably got shipped off to real prisons.

It wasn't a transfer station. For one, It's too far north and east of NCR territory to be on the way to any "real" prisons (where would they be going?). Two, the game makes it implicitly clear that the prisoners were there to be put to work clearing rocks on the nearby railroad using dynamite. That's where Samuel Cooke got his hands on extra dynamite for his breakout and subsequent escape.
 
I think Red Dead Redemption handled the World Map pretty well in that it's cities were realistically spread apart but gave you enough things to do in between your travels. Or if you wanted you could take one of several forms of transport.

I am not a professional developer. But that should be no obstacles for those which develope games to try it. I mean both Fallout 3 and Vegas have "fast travel" which is a direct route between location A and B so in other words you don't travel "in game time" to the location (like if you walk the distance and have to get around obstacles for example) but only the time you would require for a direct route. Is that more plausible ? No clue.

I think there are ways how it could work. For example if you "fast travel" on the map your character will move in a certain pattern. Trade routes for example. Or anything else that could be used for traveling like nodes on the map where the player represented as dot moves over the map. Between those nodes you could even include "random encounters". Those could be eventually avoided by the outdoorsman skill even. Or you could connect major locations with some transportation system and the rest would have to be moved by foot.
 
Something like replacing Fast Travel with route tracing? you would need to trace a route between locations or get them from specific characters like merchnts and stuff?
