Remember how the Vault Dweller writes memoirs or somesuch. Well, you could have a narrative looking back at how he saved and was cast out of his vault.
Also, the movie would have to lean really heavily on the uniquely Fallout retro-50s look, both to add comedy and distinguish it from other post-apocalyptic films.
Action scenes should be kept more grounded, rather than the current trend towards kismet-driven, dual-wielding, balls-to-the-wall stupidity that modern Hollywood leans to.

The main character would be a loner with a broken heart, driving through the wasteland in his Highwayman, smoking tumbleweed and meditating about the meaning of life and stuff like that.
Violence would be rare. Maybe a bar brawl with a Jet addicted shemale ghoul prostitute in New Reno and a random encounter with two horny gay supermutants in leather armour, but not much more. Slapstick, basically.
It would end with a big mushroom cloud send-off.
I would ask Woody Allen to write the monologues and dialogues.

I think the Fallout 1 story could work if told by the Ghoul feller in Gecko as he does in Fallout 2. He can be sitting there with your character, telling you the tale of the great hero of the wastes.
