What about starting the game in a spaceship, having the choice to blow up earth or just land ?
Of it would only change people into ghoul, not the stories, quests and interaction. Even ghoul haters wouldn't notice they have become ghouls.

rule from fo3 is really a disaster for rpg.

Not if it is Bethesda that deliver it. It would be an instant hit, with 1 000 000 000 copies sold on the first day.
They may succeed in making games, but their way of making games is more suited for MMO or GTA-like, than a Fallout or an RPG. If they make an MMO or a GTA-like, i would be insterested. (not excited, but at least interested)
(Fast Travel, Open World, lot of underdevellopped factions, nice scenery, randomized actor, sandbox behavior for npc, player house system, switchable radio stations, all would suit well for those games)
