Until you consider the fact that Bethesda doesn't give a crap about Fallout lore and will write in whatever they feel like. Kid in the Fridge is probably King of Canada or something in Fallout 5.Which is great!

All the people and enemies will be based on gimmicky stereotypes rather than anything that makes for a good story.
Now if competent writers did it, it could work.
Posters boasting about happy Canadians, graffiti against the government and other signs of fighting before the war.It would be cool to see all that Americana turned on it's head with Annexed Canada. While in the states it's all 50's music and apple pie, you would get to see mementos to the true fascist US of the old days that would make President Eden and the Enclave weep with joy.
The old world version would have been a locked down police state, with old posters all around with slogans like 'Uncle Sam is Watching You' and roadblock checkpoints with mass graves discreetly tucked away.
The American invasion could even play into the main plot: The provisional government established by America managed to survive the war and after 200 years it's about ready to collapse.
