Eh, to be fair, people have always been able to get around, granted it would be difficult after the Divide incident and the rise of Caesars' Legion, but people are clever. As long as you have the knowledge, walking across a continent isn't impossible.

Well Harold traveled all the way from NCR to DC so its not impossible. Also judging from your sonny boys age from when he was kidnapped by Kellogg and his men to how you find him later in the game(he seems to be around 60) I take it that Kellogg Cornflakes travel to the Commonwealth well before the events in New Vegas took place. Shit, probably long before Edward Sallow founded and created the Legion or even long before he was born.

I like to think that New Vegas was the final "Fuck You" Chris Avellone and J.E Sawyer gave to Bethesda as everything that was in New Vegas contradicts everything that Bethesda is trying to establish in Fallout 4.
